Report an incident

Tell us about something that went wrong — something that happened to you, something you saw, or something you have been told about.

This form is for anybody:

  • a person who receives support;
  • a family member, carer or advocate;
  • a support worker or nurse;
  • somebody at a provider organisation.

You do not have to tell us who you are

Every question about you is optional. Leave them all blank and we will still read this and still act on it. If you do leave your name off, we cannot come back to you with what we found — that is the only difference, and it is your choice to make, not ours.

Which of these is closest to you?

It changes who reads this first. It does not change whether we act on it.

In your own words. There is no right way to write this. Tell us as much or as little as you want to.

Tick this and we look at it first. If the danger is happening now, please call 000 as well.

Was a Zentra One worker involved?

"I am not sure" is a perfectly good answer and it is common.

Leave this blank if you are not sure. If your phone does not show a date picker, type the date as 2026-03-01.

A suburb, a house, a facility — whatever you can say.

If you know it. The name you know them by is fine.

For example the provider, the police, or a commission. It helps us not to duplicate what somebody else is already doing.

Your details, if you want to give them

You can leave all of this blank. We will still read what you send us and we will still act on it. If you do give us a way to reach you, we will use it to ask about this report and for nothing else.

What happens next

  • A person reads it within one business day.
  • We work out who needs to know and what has to happen.
  • If you left us a way to contact you, we are in touch within two business days.
  • You get a reference on the next page. Keep it, and you can ask us about this report later even if you did not give your name.

You can also go outside us

You do not have to come to us first, and you do not have to stop with us. The NDIS Quality and Safeguards Commission and the Aged Care Quality and Safety Commission both take reports directly from anybody.

What you write here is kept with the report and used to look into it. It is not published, and it is not repeated in an email or a website address.
